DEP Institut has participated in defining the model for follow-ups on Individual Plans for Attention
DEP Institut has participated, being entrusted by the Area of Attention to People of the Provincial Council of Barcelona and with the collaboration of the Regional Council of Maresme, on defining the model for follow-ups on the Individual Plans for Attention (PIA).
The main objectives to reach in developing the protocol for follow-up on the PIAs have been the following:
- Define, through consensus, a model for following-up on the developments of the situation regarding the beneficiaries of a PIA.
- Bring together the criteria for bringing about prioritisation circuits, channeling and timing of the follow-ups.
- Establishing the indicators for follow-up on a person’s situation in order to bring about, if be the case, a diagnosis of the needs for modification to the service.
In order to achieve the objectives proposed, a collection and review of experiences in this area has been carried out and published; six work sessions were made dynamic in order to define a model with the persons responsible for the project, done so by the Provincial Council of Barcelona and members of the Planning Commission of the Maresme Regional Council Dependency; the realisation of a pilot test applying the follow-up protocols and, lastly, the validation of the procedure and the drafting of the associated documentation;
During the first week of July, DEP Institut will carry out a seminar on the model for follow-ups on the beneficiaries of services and stipulations of the Law on Personal Autonomy and Long-term Care (LAPAD) within the framework of the XXII Summer School of the Provincial Council of Barcelona (more information http://www.diba.cat/cjs).
